*Appointments can be made in the patient's home if unable to come to the pharmacy. *It is recommended to take advantage of this valuable service and book an appointment annually. Annual medication review provides the opportunity for the patient to sit down 1-1 with the pharmacist to review detailed health profiles. We help patients better understand their medication including: - What medications they are taking - Why they are being taken - How to take them to ensure that medications are being taken as prescribed The pharmacist will provide a care plan to help patients reach their health goals and prepare a Best Possible Medication History.
