Service Description

✅We will need the following info emailed to us prior to confirming the appointment. +Name: +Date of Birth: +Gender: +Address: +Health card number, version code +Symptoms: +Onset of symptoms +History of travel in the last 14 days? +Did you receive 1st, 2nd dose of covid vaccination? +Have you been in contact with someone who tested positive? +Reason of testing (health care worker must show a valid ID as a proof of eligibility): ✅ Test results will be posted on the provincial portal (https://covid19results.ehealthontario.ca:4443/agree)   ✅The eligibility for publicly funded COVID-19 testing services has been changed. Effective October 6th, 2022, staff and students in Provincial and Demonstration schools are no longer eligible for molecular testing. Pharmacy services related to publicly funded COVID-19 testing may be offered to individuals who fall within the priority groups eligible for molecular tests: • Symptomatic people who fall into one of the following groups: o People aged 70 years and older o People aged 60 years and older who have less than three doses of COVID-19 vaccine o People who are immunocompromised o Adults aged 18 years and older who have had less than three doses of COVID-19 vaccine and have risk conditions: ▪ obesity (BMI ≥ 30kg/m2) ▪ diabetes ▪ heart disease, hypertension, congestive heart failure ▪ chronic respiratory disease, including cystic fibrosis ▪ cerebral palsy intellectual disability, sickle cell disease ▪ moderate or severe kidney disease (eGFR <60mL/min), moderate or severe liver disease (e.g., Child Pugh Class B or C cirrhosis) o Pregnant people o Staff, volunteers, residents/inpatients, essential care providers, and visitors in highest risk settings ▪ Highest risk settings include hospitals (including complex continuing care facilities and paramedic services), and congregate living settings2 with medically and socially vulnerable individuals, including, but not limited to long-term care homes, retirement homes, First Nation elder care lodges, group homes, shelters, hospices, correctional institutions, and hospital schools. o Household members of staff in highest risk settings and patient-facing health care workers and community care workers o International Agriculture Workers in congregate living settings o Patients seeking emergency medical care, People who are underhoused or experiencing homelessness o First responders, including fire, police and paramedics.
